COVID-19 has been declared a worldwide pandemic by the World Health Organization and is extremely contagious. Furthermore, as COVID-19 is contagious in nature, participants in the activities of Klinik Basketball programs may be exposed to or infected by COVID-19 and such exposure may result in personal injury, illness, permanent disability or even death.In consideration of Klinik Basketball allowing participation in their program, I agree to forever release and indemnify Klinik Basketball from any action relating to exposure or being infected by COVID-19 as a result of, or from any action, omissions or negligence, by any participant or others, including, but not limited to Klinik Basketball. Klinik Basketball has put in place prevention measures to reduce the spread of COVID-19. However, Klinik Basketball can not guarantee that participants will not become infected with COVID-19. Furthermore, participation in the activities of Klinik Basketball could increase the risk of contacting COVID-19. The parent/guardian of the registered participant, on his own behalf and on behalf of the participant, hereby agrees that Klinik Basketball will not be held responsible for any accident or loss however caused, whether negligently or intentionally, and agrees to release all instructors, staff, volunteers, and sponsors from all claims or damages which may arise as a result of/or by reason of such accident or loss.
